Hidden Cam is a simple iPhone app that lets you take pictures without anyone knowing.

The viewfinder is hidden on a screen that looks like a regular dialpad. Just tap "Call" to take a picture and save it to your iPhone's camera roll.

The screen does not change except to show a indicator when the camera is busy saving. Nobody will notice anything out of the ordinary, even if they are looking right at your screen.
